HSL accepts checks, money orders, and credit cards. Please, DO NOT PROVIDE YOUR CREDIT CARD INFORMATION when filling out this form on-line! This is not a secured site! After you fill out this form, either phone your credit card information or write it on the copy you will mail to HSL. Please note, fields with an '*' are required.

If you are a TCA, LCCA, LOTS, or TTOS member, please include your member number with your order. Money orders and checks that include a valid club member number will ship immediately. Other checks are subject to a seven day hold.

Total price will be $50 times the order quantity plus $5 shipping and handling. Illinois residents please add 6.6% sales tax ($3.30 per unit).
